The information requested as part of this program is authorized by California Health and Safety Code sections 120125, 120130, and 100325, and Title 17 of the California Code of Regulations section 2505. Submission of the information is voluntary. We are collecting this information to evaluate the messaging presented on the CA Notify website. The information will be used to identify improvements and to ensure all instructions related to the CA Notify system are clear and understandable.
