Ambulation Study Interest Form

Please complete the survey below to participate in the Ambulation Study which benefits the UCSF Orthotic and Prosthetic Centers. If you are eligible, you will be contacted via email about your availability. Your 1-2 hour data collection session will occur in the Human Performance Center at 1500 Owens Street, San Francisco and will typically occur on Friday afternoons. Required attire will be athletic shorts, a tank top, a sports bra for women. This will allow for easy marker placement for movement capture. By filling out this form you will be indicating consent to be contacted about study participation. However, you may choose to opt out at any time. Thank you!
